Tadalafil Lilly

This is a summary of the European public assessment report (EPAR) for Tadalafil Lilly. It explains how the Agency assessed the medicine to recommend its authorisation in the EU and its conditions of use. It is not intended to provide practical advice on how to use Tadalafil Lilly.

For practical information about using Tadalafil Lilly, patients should read the package leaflet or contact their doctor or pharmacist.

Tadalafil Lilly is a medicine used to treat men with erectile dysfunction (sometimes called impotence).

It can also be used in men to treat the signs and symptoms of benign prostatic hyperplasia (enlarged prostate gland that is not cancerous, which leads to problems with the flow of urine).

Tadalafil Lilly contains the active substance tadalafil and is the same as Cialis, which is already authorised in the European Union (EU). The company that makes Cialis has agreed that its scientific data can be used for Tadalafil Lilly (‘informed consent’).

Tadalafil Lilly is available as tablets (2.5, 5, 10 and 20 mg) to be taken by mouth. For erectile dysfunction, the usual dose is 10 mg at least 30 minutes before sexual activity. The dose may be increased to 20 mg if the 10 mg dose is not effective. Men who intend to use Tadalafil Lilly frequently (twice a week or more) can take a lower dose (5 or 2.5 mg) once a day, based on the doctor’s judgement. The medicine should be taken around the same time every day and the need for the once-a-day dosing should be assessed regularly.

In men with benign prostatic hyperplasia, or both benign prostatic hyperplasia and erectile dysfunction, the recommended dose is 5 mg once a day.

The active substance of Tadalafil Lilly, tadalafil, belongs to a group of medicines called phosphodiesterase type 5 (PDE5) inhibitors. It works by blocking the phosphodiesterase enzyme, which normally breaks down a substance known as cyclic guanosine monophosphate (cGMP).

During normal sexual stimulation, cGMP is produced in the penis, where it causes the muscle in the spongy tissue of the penis (the corpora cavernosa) to relax, allowing the flow of blood into the corpora, producing the erection. By blocking the breakdown of cGMP, Tadalafil Lilly restores erectile function during sexual stimulation and also improves the blood flow to, and relaxes the muscles of, the prostate and bladder. This may reduce the problems with the flow of urine which are symptoms of benign prostatic hyperplasia.

Tadalafil Lilly has been shown to improve the ability to get and maintain an erection in nine main studies involving over 2,000 men with erectile dysfunction. All the studies compared Tadalafil Lilly with placebo (a dummy treatment) and measured improvements using questionnaires completed before and after treatment.

Six of the studies included 1,328 patients who took the medicine before sexual activity. Results for one of the questionnaires, in which men rated their condition on a scale of 0 (indicating severe erectile dysfunction) to 30 (no erectile dysfunction), showed that patients improved from about 15 before treatment, to 23 and 25 after treatment with Tadalafil Lilly 10 and 20 mg, respectively. The other three studies included 853 patients taking Tadalafil Lilly once a day at doses of 2.5 or 5 mg. Results also reported improved erections with Tadalafil Lilly compared with those taking placebo.

Tadalafil Lilly has also been shown to improve the symptoms of benign prostatic hyperplasia as measured using the international prostate symptom score (IPSS), which ranges from 0 (no symptoms) to 35 (severe symptoms). In four main studies involving 1,500 patients, some of whom also had erectile dysfunction, patients taking Tadalafil Lilly at a dose of 5 mg had greater reductions in IPSS (4.8 to 6.3 points) after 12 weeks than those taking placebo (2.2 to 4.4 points).

The most common side effects with Tadalafil Lilly are headache, indigestion, back pain and muscle pain, which are more common at higher doses. For the full list of all side effects with Tadalafil Lilly, see the package leaflet.

Tadalafil Lilly must not be used in men with certain cardiovascular (heart and blood vessel) conditions or when sexual activity is inadvisable. It must also not be taken by patients who have ever had loss of vision in one eye because of a problem with blood flow to the nerve in the eye (non-arteritic anterior ischemic optic neuropathy, NAION). Tadalafil Lilly must not be taken with nitrates (a type of medicine used for angina) or medicines of the class ‘guanylate cyclase stimulators’ such as riociguat (a medicine for high blood pressure in the vessels supplying the lungs, known as pulmonary hypertension). As shown in studies, Tadalafil Lilly is effective at improving the ability to get and maintain erections and at relieving the symptoms of benign prostatic hyperplasia. The side effects seen with the medicine are considered manageable. The Agency’s Committee for Medicinal Products for Human Use (CHMP) therefore concluded that Tadalafil Lilly’s benefits are greater than its risks and recommended that it be approved for use in the EU.

The European Commission granted a marketing authorisation valid throughout the European Union for Tadalafil Lilly on 22 March 2017.

Cialis (Tadalafil) is a prescription medication for the treatment of erectile dysfunction (ED) in men. This is when a man cannot get, or keep a hard, erect penis suitable for sexual activity. It is available in 10mg and 20mg tablets and is taken about 1 hour prior to sexual activity.

Once a day dosing of Cialis (2.5mg and 5mg tablets) may be useful to men who anticipate having sexual activity two or more times per week. The recommended dose is one 5 mg tablet taken once a day at approximately the same time of the day. Your doctor may adjust the dose to 2.5 mg based on your response to Cialis.

When taken once a day Cialis allows you to obtain an erection, when sexually stimulated, at any time point during the 24 hours of the day.

Cialis helps increase blood flow to the penis, so it can help men with ED get and keep an erection that lasts long enough for sexual activity. Once a man has completed sexual activity, blood flow to his penis should decrease and his erection should go away.

Cialis is a proven, effective treatment that works for many men. If you benefit then your partner will benefit too.

Cialis (Tadalafil) belongs to a class of drugs called “PDE5 inhibitors.” It works by increasing blood flow to the penis to help improve erectile function. The active ingredient in Cialis works specifically on the chain of events that occurs in the penis during arousal, when two large chambers in a man’s penis fill with blood.

Take it about an hour before sex (at least 30 minutes before).
Cialis may be taken with or without food.

It starts working within about 30 minutes and can last for up to 36 hours. Take Cialis no more than once in 48 hours.

Always take Cialis exactly as your doctor has told you. You should check with your doctor or pharmacist if you are not sure.

The recommended starting dose is one 10 mg tablet before sexual activity. If the effect of this dose is too weak your doctor may increase the dose to 20 mg. Cialis tablets are for oral use. Swallow the tablet whole with some water

Drinking alcohol may affect your ability to get an erection. Drinking alcohol may temporarily lower your blood pressure. If you have taken or are planning to take Cialis, avoid excessive drinking (blood alcohol level of 0.08% or greater), since this may increase the risk of dizziness when standing up.

* You have a serious liver problem.
* You have a severe kidney problem.

It is not known if Cialis is effective in patients who have undergone pelvic surgery or radical non-nerve-sparing prostatectomy.

If you experience sudden decrease or loss of vision, stop taking Cialis and contact your doctor immediately.

Cialis is not intended for use by women or by adolescents under the age of 18.

Please tell your doctor if you are taking or have recently taken any other medicine, including medicines obtained without prescription, because they might interact.

This is particularly important if you are treated with nitrates as you should not take Cialis if you are taking these medicines.

A type of medicine called an alpha blocker is sometimes used to treat high blood pressure and enlarged prostate. Tell your doctor if you are being treated for either of these conditions or if you take other medicines to treat high blood pressure.

If you are taking medicines that can inhibit an enzyme called CYP3A4 (for example ketoconazole or protease inhibitors for treatment of HIV) the frequency of side effects might increase.

Do not take Cialis with other medicines if your doctor tells you that you may not.

You should not use Cialis together with any other treatments for erectile dysfunction.

Some men taking Cialis in clinical studies have reported dizziness. Check carefully how you react to the medicines before driving or using any machinery.
Important information about some of the ingredients of Cialis:

Cialis contains lactose. If you have been told by your doctor that you have an intolerance to some sugars, contact your doctor before taking this medicinal product.

Like all medicines, Cialis can cause side effects, although not everybody gets them. These effects are normally mild to moderate in nature.

In this leaflet, when a side effect is described as “very common” this means that it has been reported in at least 1 in 10 patients taking the medicine. When a side effect is described as “common” this means that it has been reported in more than 1 in every 100 patients but less than 1 in every 10 patients. When a side effect is described as “uncommon”, this means it has been reported in more than 1 in every 1,000 patients, but less than 1 in every 100 patients. When a side effect is described as “rare”, this means it has been reported in more than 1 in every 10,000 patients, but less than 1 in every 1,000 patients.

Very commonly reported side effect in patients taking Cialis were headache and indigestion.

Commonly reported side effects in patients taking Cialis include back pain, muscle aches, facial flushing, nasal congestion, dizziness, pounding heartbeat sensation, abdominal pain and reflux.

Uncommon side effects are allergic reactions including rashes and hives, blurred vision, swelling of the eyelids, eye pain, red eyes, increased sweating, nose bleeds, a fast heart rate, high blood pressure, low blood pressure and chest pain. In case of chest pain occurring during or after sexual activity you should NOT use nitrates but you should seek immediate medical assistance.

Rare side effects in patients taking Cialis include fainting, migraine and swelling of the face.

In rare instances it is possible that a prolonged and possibly painful erection may occur after taking Cialis. If you have such an erection, which lasts continuously for more than 4 hours, you should contact a doctor immediately.

Heart attack and stroke have also been reported rarely in men taking Cialis. Most, but not all of these men had known heart problems before taking this medicine. It is not possible to determine whether these events were directly related to Cialis.

Partial, sudden, temporary, or permanent decrease or loss of vision in one or both eyes has been rarely reported.

Some additional side effects have been reported in men taking Cialis that were not seen in clinical trials and their incidence is unknown. These include seizures and passing memory loss, some disorders affecting blood flow to the eyes, irregular heartbeats and angina, serious skin rashes and sudden cardiac death. Sudden decrease or loss of hearing has been reported.

Effects were seen in one animal species that might indicate impairment of fertility. Subsequent studies in man suggest that this effect is unlikely in humans, although a decrease in sperm concentration was seen in some men.

If any of the side effects gets serious, or if you notice any side effects not listed in this leaflet, please tell your doctor or pharmacist.

What is Cialis?

Cialis is a drug that is used for erectile dysfunction. The active ingredient in Cialis is tadalafil and it belongs to the class of drugs known as the PDE-5 inhibitors. This is the same class of drugs as Viagra (sildenafil), except Cialis has a longer duration of action than Viagra.

Cialis is available as two different options:

Cialis Daily is available in 2.5mg and 5mg tablets. Cialis Daily is taken every day as opposed to taking it before sexual activity. This means that you are prepared for whenever the opportunity for sexual activity presents itself.

Cialis 36 hour (10mg & 20mg)
Cialis 36 hour is taken 30-60 minutes before sexual activity and has a duration of action for up to 36 hours. Although it is not taken every day, men can still benefit from its long window of action.

How does Cialis work?

Cialis works in a similar way to Viagra. Upon sexual stimulation, your nervous system releases nitric oxide. This triggers the production of a messenger called cGMP, which causes blood to flow to your penis causing an erection. The enzyme PDE-5 causes the breakdown of cGMP, so that an erection cannot occurr. Cialis blocks PDE-5 so that it does not breakdown cGMP, therefore cGMP is available allowing blood to flow to your penis. This can only happen when you are sexually excited.

How long does Cialis take to work?

Cialis Daily (2.5mg & 5mg)
Cialis Daily takes effect from around 3-5 days.

Cialis 36 hour (10mg & 20mg)
Cialis 10mg & 20mg should be taken 30-60 minutes before sexual activity

How long does Cialis last?

What dose of Cialis should I take?

Cialis daily is available for those looking for a pronlogued effect, allowing for spontaneous sexual activity. It is available as two strengths, 2.5mg and 5mg. Most men start by taking the 5mg tablet. If you find that you are experiencing side-effects, you can reduce the dose to 2.5mg.

Cialis 36 hour has a long duration of action and is available for men not wanting to take a tablet every day. It is available in two strengths, 10mg and 20mg. It is advised to start with the 10mg tablet, in order to reduce the possibility of side-effects. If after several separate attempts, you are not experiencing the desired effect, you can increase to 20mg.

What is the difference between Cialis and Tadalafil?

Cialis is a brand name, which contains the active ingredient, Tadalafil. Whilst the appearance of the tablet may differ, they are medically the same and contain the same ingredient, therefore having the same effect. Before manufacturers of generic medication are allowed to introduce their drugs to the market, they must be able to demonstrate that the generic form is bioequivilant (pharmaceutically the same as) its branded counterpart and meet the same strict manufacturing standards.

Why is Cialis more expensive than Tadalafil?

The pharmaceutical company Eli Lilly developed Cialis in 2003. They held the patent (which is the sole right to exclude other companies from making the drug) until November 2017. When this patent expired, other companies began to manufacture the drug under its generic name Tadalafil, at a fraction of the price. This is because these companies have not needed to invest into the research and development or marketing of the drug, meaning they have no expenses to claw back. As Cialis was the only 36 hour erectile dysfunction treatment on the market for 14 years, they faced no competition, which is another reason as to why it was priced so highly. As generic tadalafil’s popularity increases, it is expected that the price of Cialis will also come down.
There are now a number of companies manufacturing Tadalafil. Increased competition means that tadalafil is available at a cheap price.

Is Cialis better than Viagra?

Both treatments have different characteristics and effect different individuals in different ways. They have both been proven to be effective treatments in a number of men. Cialis appears to be more appealing due to its faster onset of action (can be 15-20 minutes as opposed to 30-60 minutes for Viagra), it’s longer duration of action (36 hours or continuous effect as opposed to up to 4 hours for Viagra) and it’s lower side-effect profile. However, many find that Viagra works fine for them and are happy with it. It depends on you as an individual and what your needs are and more importantly, which treatment works best for you.

What if Cialis is not working for me?

It is important try Cialis a few times in order properly asses whether you need to increase the dose or try another treatment. Try taking it a full hour before sexual activity. Although Cialis can be taken shortly after a meal, try waiting atleast two hours before you take it. Cialis will only work where there is sexual exctiement. Keep in mind that without arousal, Cialis will not work. Try to keep a clear head and relax. If you find that Cialis is still not providing you with the desired outcome, try incresaing the dose. If you have already tried taking the maximum dose on several separate attempts, it may be worth speaking to your GP or calling our helpline. ​

Is Cialis safe?

If taken correctly, Cialis shouldn’t have any serious or life threatening effects. It is important to answer our medical questionnaire as truthfully and accurately as you can in order to minimise any health risks.

Can I drink alcohol with Cialis?

Moderate alcohol consumption whilst taking Cialis is not likely to cause any interactions or cause any health risks. However, excessive consumption may lead to a drop in blood pressure, resulting in headache, dizziness, fainting or palpitations.
Excessive alcohol consumption can also effect the amount of blood that is available to your penis, therefore making erectile dysfunction worse.
